Sports physicals are required yearly for children participating in school athletics. Our office will complete your child’s sports physical form if your child has had a health maintenance exam in the past year. Please fill out page 1 of the Florida sports physical form and drop it by the office. Copies of the forms are available here.Please allow us 24 hours to complete the form.

If your child has not had a well check in the past year or their sports physical expires before the end of the school year, it is often at the discretion of the coach if a new sports physical is required.

To better serve you, please bring to your child’s sports physical any vaccine documentation (tetanus in particular) and the Florida Sport Physical form. Please also fill out page one of the form before your visit.
