Appointment Scheduling Policies

To better serve our patients, Mount Dora Pediatrics is an appointment clinic. Calling to schedule an appointment allows us to reduce wait time and prevent “bottle-necking” in the office. We occasionally see established patients on a “walk in” basis; however, we recommend calling ahead whenever possible as it may reduce wait time.

You can "request an appointment" on line via the link in the upper tabs. This however is only a request and not an appointment confirmation. The office will contact you via phone to schedule and confirm the appointment within 48 hour of your on line request.  Please provide a reliable daytime phone. If your child needs an appointment sooner than 48 hours, please call the office to arrange this at (352) 383-0624.

First Time Office Visit Patients

If the patient is ill, the visit will be considered a sick visit. If the patient is not ill and has not had a well check within the past year, the visit is considered a well check.

Please complete the initial forms and paperwork before your appointment or plan to arrive 15 to 20 minutes early to provide the information (Forms link). Failure to complete forms before arrival may result in other patients being seen before your scheduled appointment.

Scheduling Sick Visits

Most sick visit appointments may be made for the same day, as openings are included in the daily schedule to accommodate our sick patients. In the rare instance when a same day appointment is unavailable, the nurse will be able to discuss your child’s problem. If the nurse determines that the child should be seen that day, we will see the patient. Should your child require after hours care, please call the office number to reach Dr. Baker.

Scheduling a Well Visit or Annual Physical Examination

Mount Dora Pediatrics adheres to the policies of the American Academy of Pediatricians, the U.S. Preventive Services Task Force, Bright Futures and the Institute for Clinical Systems Improvement (ICSI) regarding the need for routine health maintenance exams. These guidelines apply to patients with and without illness. Lake County Public Schools also require physicals for entrance into K-5 and Middle School. Day Cares and preschools also require yearly health maintenance exams.

Well checks are a vital time to address new and ongoing medical issues and to evaluate growth and development (height, weight, motor, language, social, cognitive/learning, behavior). It is also an important time to screen medical issues that have minimal or no present symptoms but may lead to later complications such as hearing loss, hypertension, urinary, cardiovascular or orthopedic problems as well as obesity, sleep disorders and others. Well checks also are important for assisting with normal concerns and for health maintenance and prevention. Vaccinations represent only a small portion of the appointment.

Newborn Physicals: in the first week if there were medical issues in the nursery or if the child was not seen by Dr. Baker in the nursery then 

Physicals At 2 Weeks , 2 Months , 4 Months , 6 Months , 9 Months, 12 Months , 15 Months , 18 Months,  24 Months , 36 Months , 4 Year,  5 Years, 6 Years , 8 Years,  10 Years, 11 Years,  12 Years, 14 Years, 15 Year, 16 Years, 17 Years 18 Years

Sports Physicals Scheduling

Sports physicals also are required yearly for children participating in school athletics. Our office will complete your child’s sports physical form if your child has had a health maintenance exam in the past year. Please fill out page 1 of the Florida sports physical form and drop it by the office. Copies of the forms are available at http://www.neaseathletics.com/EL02_physical.pdf Please allow us 24 hours to complete the form.

If your child has not had a well check in the past year or their sports physical expires before the end of the school year, it is often at the discretion of the coach if a new sports physical is required.

To better serve you, please bring to your child’s sports physical any vaccine documentation (tetanus in particular) and the Florida Sport Physical form. Please also fill out page one of the form before your visit.

Developmental/Learning or School-related Appointments

If you have developmental concerns about your child, i.e. Autism, ADHD, developmental delays, etc., please allow one hour for the initial appointment. Also, please check with your insurance provider before the visit to determine if there is mental health and/or developmental coverage and whether the insurance plan requires a referral to their mental health network. Developmental appointments are scheduled for 11 a.m. and 4 p.m.

The initial developmental evaluation may last longer than one hour. The second developmental appointment may require 30 to 60 minutes. Subsequent developmental appointments generally do not require more than 30 minutes. Please bring an additional adult if more than one child is present at the time of the appointment. Well check appointments are not performed during developmental appointments.

Prenatal appointments

A prenatal appointment is scheduled in advance and there is no charge for the visit. The appointment usually lasts between 15 to 30 minutes. Please prepare any questions before the appointment.

After Hours Phone Consults

Dr. Baker is available for after hour consults seven days a week for urgent and emergency issues. When Dr. Baker is away from the office, coverage will be provided by other area FPMG pediatricians.

The primary purpose of phone consultations is to assist with medical questions and to determine if additional evaluation is needed and when it should be performed. In the best interest of the child, after hour consults are not appropriate for diagnosis and treatment. Please call 352-383-0624 (main office line) to arrange an after hour phone consult.
